Semi Independent Living

When a client has acquired the skills needed to live without continual supervision, they are ready for our Semi-Independent Living Program. Here clients may explore their abilities in the community while still benefiting from contact with Titleworth Healthcare's rehabilitation staff and professional therapy team.

Comprised of small homes and apartments, this multi-tiered, transitional program offers a safe and supportive environment as clients take critical steps toward independence. The graduated levels within the program provide the level of support and independence appropriate to each client's unique needs. The Titleworth Healthcare staff is located nearby to provide assistance as needed.

Program features include:

  • On-site rehabilitation treatment team
  • Psychologists, mental health professionals and rehabilitation nurses
  • Case managers and nurses on call, 24 hours a day, seven days a week
  • Therapeutic groups focusing on client awareness, orientation, social skills, brain injury education, independent living skills and community reintegration
  • Practical training in problem solving, financial responsibility and activities of daily living such as housekeeping, using public transportation, college enrollment, etc.
  • Availability of accessible public transportation
  • Access to schools, jobs and educational programs
