Our Programs include:
Active Transitional Therapy - highly individualized treatment that is aggressive and focused on advancing through the various stages of recovery. Core therapies (occupational therapy, physical therapy and speech and language pathology, for example) are rigorously pursued with an interdisciplinary approach. To increase independence and expedite recovery, individual therapies are supplemented with group sessions, home programs and community reintegration activities.
Medical Care - a specialized program delivered in our comfortable residential settings. Our team of medical professionals and neuro-therapists provide care and support as directed by the client’s GP. Specially trained rehabilitation assistants provide 24-hour care. Our Medical Program makes the management of pre-injury conditions easier. In addition to care, Titleworth Healthcare provides medical training and health education for those seeking an independent lifestyle.
Semi-Independent Living - When an individual has the skills needed to live without continual supervision, they are ready for our Semi-Independent Living Program. Here clients explore their abilities in the community while still benefiting from contact with Titleworth Healthcare’s rehabilitation staff and professional therapy team. Comprised of community homes and apartments, this multi-tiered, transitional program offers a safe and supportive environment as clients take critical steps toward their own independence. The graduated levels within the program provide the level of support and independence appropriate to each client’s unique needs.
Supported Living - Not every individual is able to progress to independent living. For this reason, Titleworth Healthcare provides either a residential setting or community home environment for those who will require supervision and supportive care for extended periods of time. To enjoy the highest possible quality of life, clients participate in therapeutic groups, frequent community outings and recreational activities and are encouraged to pursue personal interests.
Respite Care - Families who provide at-home care for an individual with a brain or spinal cord injury may occasionally need supplemental, short-term care for their loved one. Titleworth Healthcare welcomes these visits, and clients look forward to participating in Titleworth Healthcare outings and enjoy the company of peers.
Outreach Service (homecare) - Individuals who are living in their own homes can call on Titleworth Healthcare to provide ongoing therapeutic rehabilitation services. Our professionals can help with personal care and activities of daily living - such as meal preparation, shopping, money management and community access to ensure successful home living.
Vocational Training - For restoring self-confidence and self-respect, nothing compares to a paycheck and a job well done. Titleworth Healthcare’s Vocational Program provides a variety of environments from structured settings to community-based employment, allowing our clients to learn new skills or regain old ones.
