Neuro Case Management

Titleworth Healthcare is proud to work alongside Neuro Case Management who provide case management services that are designed to meet the unique needs of individuals who have sustained a neurological illness or injury.

BABICM (the British Association of Brain Injury Case Managers) defines Case Management as a process devoted to the coordination, rehabilitation, care and support of people with complex and clinical needs. The provision of services to clients aims to facilitate their independence through the assessing, facilitating, planning and advocating of their needs on an individualistic basis. This approach thus ensures that management is "client" focused (ensuring that the most appropriate services for the client are accessed) and that continuity, consistency and structure are rigorously maintained.

The approach of Neuro Case Management is to identify and build upon client's strengths through the establishment of both short and long-term goals for their rehabilitation. The functional areas which are assessed prior to beginning case management services include: medical status, physical and cognitive functioning, independent living skills, emotional and behavioural skills and recreational and vocational areas of their life. Following the case management assessment through liaisons with the MDT and the client and their representatives, the specific goals, recommendations, and resources are identified, co-ordinated and initiated.

Case management services can be short or long-term and often involve ongoing coordination and communication with the various treatment providers and family and funders alike. Through the provision of case management services there is ongoing advocacy support and consistent follow-ups to ensure that the client's needs are fully met through the rehabilitation process.

Neuro Case Management services are provided to individuals who are referred to us from insurance companies, solicitors, PCTs and Social Services departments, professionals in the field, and by family members and individuals who have sustained the injury or illness themselves.
