Titleworth Healthcare has developed a uniquely successful Managed Pathway Model that ensures effective rehabilitation results for its clients for all stages of their neuro-recovery. With strengths in all areas of the rehabilitation model Titleworth is best placed to provide its clients with effective and result-driven services thus allowing them to develop their independent skills more efficiently. The speedy progress through the recovery process aids our clients in living more independently and with increased confidence and control in their daily lives.

In the Managed Pathway Program, clients are engaged in highly individualized treatment, which is poignant and focused on advancing through the various stages of brain injury recovery. Core therapies (neuro-psychology, occupational therapy, physical therapy and speech and language therapy, for example) are rigorously pursued with an interdisciplinary approach and fully overseen by individual ABI Case Managers. In order to increase independence and precipitate recovery, individual therapies are supplemented with group sessions, home programs and community reintegration activities. The pace of the Program calls for frequent progress reporting, ongoing care plan review and revision and regular contact with all stakeholders. The goal of this program is to progress individuals as quickly as possible to their highest functional ability and, whenever possible, to return them to their own homes either independently or with support.

Titleworth's Managed Pathway Program can be entered into at any time and covers all areas of rehabilitation from the acute stage through to the transition to independent living. Titleworth provides clients with a variety of purpose-built and award-winning residential environments for each stage of the Pathway (Acute, Post Acute, Transitional, Supported Living, Independent-through home support) where each resident is managed throughout by a highly skilled Case Manager and specialist neuro-therapist team. All of our clients therefore, are fully enabled to enter seamlessly and with constant support through each stage of their own recovery.
