Created as the core of Titleworth Neuro’s rehabilitation services, our Managed Pathway Programme provides therapies such as neuro-psychology, occupational therapy, physical therapy and speech and language therapy, some or all of which can be within residential accommodation. Service users are given a tailor-made rehab programme, with carers working in constant collaboration with them and their families. All individual rehabilitation programmes are overseen by ABI Case Managers, and therapies are supplemented with group sessions, home programmes and community reintegration activities.
Titleworth Neuro's Managed Pathway Programme is designed to provide seamless support in all areas of neuro rehabilitation, and can be entered into at any stage of recovery; these stages are defined as Acute, Post Acute, Transitional, Supported Living and Independent through home support. Within our purpose-built, luxurious, modern and award-winning residential accommodation, each resident is managed throughout by a highly skilled Case Manager and specialist neuro-therapist team, then supported back into their own homes where possible.
